SO i realized it is always always better to use 2nd wind in combat rather than wait to use the healing surge outside of combat. the reason, as long as you take less damage than your max healing surge (taken 10 dmg or more when your surge only heals for 9 or less). I don't know if players have thought about this, but second wind is mainly good because once per encounter it adds the +2 to all defenses for a round. So, instead of waiting outside of combat to heal back up, you might as well use that once per encounter 2nd wind and gain those extra defenses instead of just wasting the surge outside of combat and not getting the defense bonuses.
